This vintage form is from the New York bridal and millinery district. This form can easily be decorated with lace trims, appliques, beads, feathers, or rhinestones (or all of the above!).  The edge is stabilized with covered wire. Make a bridal headpiece or fascinator. This headpiece has the distinctive extra back piece for stability, which can be embellished for extra interest. This piece needs some TLC (steam and extra sizing to restore the shape).
